- The people, vehicles and insurance details involved.
- A description of what happened, supported by photos and a sketch.
- Relevant statements, witness details and signatures.
Your insurer may ask for a particular form or additional information. Check its reporting instructions before submitting your documents.

Across languages and devices
The other party can enter details without installing the app.
A QR invitation opens an App Clip on a compatible iPhone or a browser form on other devices. This lets the other party enter their details without installing the full app.
Entering details is different from creating a complete report or receiving a report copy. The direct handover of a report copy described on this website uses an iPhone with the app or App Clip. The App Clip does not provide the full report-creation and insurer-submission workflow.
